Clinical Education Manager LVN Mobile Wound Care

Job description

This dual-role field position supports wound care excellence and regulatory compliance by training providers (MDs, DOs, NPs, PAs), conducting documentation audits, and serving as a liaison to SNF and post-acute leaders. All duties are performed within the LPN scope of practice as defined by the New York State Education Department’s Office of the Professions, and in collaboration with licensed providers. You’ll help ensure alignment with CMS and New York Department of Public Health (DPH) standards, including F-tag documentation and QAPI efforts.

 

Key Responsibilities
Clinical Training & Onboarding

  • Provide wound documentation training (e.g., staging, measurements, narrative writing)

  • Lead in-person onboarding and clinical shadowing for new mobile providers

  • Conduct in-services, charting labs, and mock audits at SNFs and HHAs

  • Reinforce F-tag compliance (F684, F686, F657, F838/F865) and align training with NY DPH guidelines

Audit & Documentation Coaching

  • Review provider documentation for accuracy, completeness, and compliance

  • Identify risk areas and guide providers through remediation

  • Assist SNFs and HHAs in survey prep and internal documentation practices

  • Contribute to QAPI tracking and documentation coaching initiatives

Client Relationship Support

  • Act as a liaison between OWP and assigned partner facilities

  • Escalate concerns related to provider documentation or client needs

  • Support DONs, ADONs, and nursing teams in maintaining wound care and survey readiness standards

Job requirements

Minimum Qualifications
Education & Licensure

  • Associate degree in nursing or healthcare-related field required

  • Bachelor’s degree (BSN or equivalent) preferred

  • Active, unrestricted LPN license in New York

  • Valid New York driver’s license and clean MVR

  • Proof of personal vehicle insurance

Experience

  • 3+ years of clinical wound care experience in SNF, LTC, or post-acute settings

  • 1+ year in clinical education, onboarding, documentation coaching, or survey preparation

  • Familiarity with New York DPH and CMS survey standards

  • Experience influencing compliance through training and audit support

Technical Skills

  • Proficient in EMRs (PointClickCare, MatrixCare, etc.)

  • Skilled in Google Workspace (Docs, Sheets, Gmail, Slides)

  • Experience using Slack or mobile field tools

  • Typing speed of 45–60 WPM

Preferred Certifications

  • Wound care certification (e.g., WCC, WOCN, CWS)

  • QAPI or quality management experience

 

Technical & Physical Requirements
Driving & Travel:

  • Frequent travel required within a 50-mile service area

  • Must have reliable personal vehicle, valid driver’s license, and insurance

  • Travel between assigned work locations during the workday is compensable time and will be paid in accordance with New York wage and hour laws

Mobility & Ergonomics:

  • Able to stand/walk for extended periods in SNF and clinical environments

  • Able to lift, carry, or transport up to 25 lbs of equipment or materials

  • Regular use of laptops, tablets, and mobile devices in a variety of field conditions

Position Details

  • Part-time (1–3 shifts per week)

  • May transition to full-time based on regional demand

  • Flexible field-based schedule; some evenings/weekends may be required during onboarding or survey support

  • Not a remote position

 

Pre-Hire Requirements

  • Background check conducted in compliance with New York Correction Law Article 23-A

  • I-9 employment eligibility verification

  • MVR screening

  • Drug screen (if required by partnering facilities)

  • Clinical reference check

  • Active NY LPN license verification

  • Proof of current TB screening and immunization records as required by New York State Public Health Law and partner facility policy
