Job description

This team brings preventive and primary care directly to where patients are most comfortable their homes, community sites, skilled nursing facilities, and pop-up clinics. As a Field Nurse Practitioner, you’ll perform comprehensive assessments, manage acute and chronic conditions, coordinate care, and help reduce hospital readmissions. 

Travel Requirements:
This is a field-based role with local travel within your service area. Most visits are nearby, but some days may involve extended driving up to about 120–125 miles spread throughout the day (about 2–3 hours total drive time). Mileage reimbursement is provided.

Key Responsibilities

Provide primary and preventive care to diverse populations (adult, pediatric, women’s health, geriatric)
Perform full history and physical exams, identify gaps in care, and develop treatment plans
Order/interprets labs & imaging, request specialist consults, and prescribe medications
Conduct visits in homes, nursing facilities, community clinics, and via approved telehealth
Collaborate with other NPs and actively participate in team meetings
Document care promptly and accurately in EMR

Schedule Options

Option 1: Standard 8-hour days (Mon–Sat)

  • 3 days: 8AM–5PM

  • 2 days: 10AM–7PM

  • 6–7 patients/day

Option 2: 4 x 10-hour days (Mon–Sat rotation)

  • 8AM–7PM

  • 8–9 patients/day

  • One Saturday/month required

Job requirements

Minimum Requirements

  • Master’s degree in Family Health (ANCC/AANP accredited)

  • Active & unrestricted NP license

  • National board certification (ANCC or AANP)

  • Valid driver’s license and willingness to travel

  • EMR proficiency and strong documentation skills

  • Basic Life Support (BLS) certification

Preferred Qualifications

  • 2–5 years as a Nurse Practitioner

  • Home or public health experience

  • Bilingual fluency (preferred)

  • Experience with immunizations and point-of-care testing
