This NP opportunity is a facility-based team of medical providers who provide medical services to patients living in long term care facilities. Our clients strive to give quality care to residents while working together with the facility to carry out the best care possible. Comp: $115,000.000-$130,000.00 depending on location and experience

Travel Requirements:
This is a field-based role with local travel within your service area to LTC, SNF and ALF. Most visits are nearby, Mileage reimbursement is provided.

Key Responsibilities

  • Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

  • Must be comfortable staging pressure injuries according to the NPIAP

  • Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

  • Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

  • Strong preference to having wound certification or be open to getting certified 

  • Experience working in a long-term care setting.

  • Ability to work with an EMR.

  • Ability to work within an interdisciplinary care team to coordinate patient care.

  • Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

  • Manage Residents with acute change in condition.

  • Advanced care planning and chronic care management.

  • Prescribe appropriate medications, including controlled substances in accordance with State Law.

  • Order, review and/or follow-up with treatments and diagnostic testing.

  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

  • and/or the care plan.

  • Collaborate with primary care physician.

Job requirements

Minimum Requirements

  • Master’s degree in Family Health (ANCC/AANP accredited)

  • Active & unrestricted NP license

  • National board certification (ANCC or AANP)

  • Valid driver’s license and willingness to travel

  • EMR proficiency and strong documentation skills

  • Basic Life Support (BLS) certification

  • Ability to manage patients with advanced illness and multiple chronic conditions.

  • Ability to work within an interdisciplinary care team to coordinate patient care.

  • Wound certificate preferred or opened to getting certified

Preferred Qualifications

  • 2–5 years as a Nurse Practitioner

  • Home or public health experience

  • Bilingual fluency (preferred)

  • Experience with immunizations and point-of-care testing
